Vision Plan
The Vision Plan is included free of charge for every member of the QCD "RED" Program. This program features savings through Avesis nationwide. "Enhanced" Vision Programs available through Avesis (additional cost).
The QCD "RED PLUS" Program combines the QCD Managed Cost "RED" Program (NOT an insurance plan) and a Scheduled Insurance Plan provided by Madison National Life Insurance Company, Home Office: Madison, WI. Administrative services are provided by, GroupLink, Inc., Home Office: Indianapolis, IN.
