Overview

The QCD "Red Plus" Program combines the QCD Managed Cost "Red" Program (not an insurance plan) and a Scheduled Insurance Plan provided by Madison National Life Insurance Company, Home Office: Madison, WI.

Administrative services are provided by, Independence Holding Company.

The "Red Plus" Dental & Vision Benefit Program

All of the benefits of the "Red" with 100% reimbursement for cleanings, x-rays and exams twice per year.

Vision Plan

A discount vision is included free of charge for every member of the QCD "Red Plus" Program. This program features savings through Davis Vision nationwide. "Enhanced" Vision Programs available through Davis Vision (additional cost).
