The "Blue" Dental and Vision Plan - Traditional Insurance Plan

The QCD "Blue" Program features:

  • Freedom to Choose Any Dentist - No Network
  • Immediate Coverage for Preventative Dental Services
  • Claims Paid at Percentage of Usual & Customary*
  • Optional Orthodontia Coverage**
  • "Simple Savings" Vision Benefits available through Davis Vision (no additional cost)
  • "Enhanced" Vision benefits available through Davis Vision (additional cost)

* Reasonable and customary fees are charges that do not exceed the general level of charges being made by other providers of dental services in the geographic area where the charge is incurred.

** Orthodontia coverage offered at an additional cost.
