Here are the answers to some frequently asked questions and services that we provide exclusively for our members include:
How do I use my QCD Dental & Vision Benefit Program?
The QCD Dental & Vision Benefit Program offers you and your family comprehensive coverage in a cost-effective manner. Simply follow these steps to put the program to work!- Select a QCD affiliated dentist from our large network in your area.
- Contact the dentist�s office and identify yourself when scheduling your appointment. Please remember that the QCD benefits are only available through a QCD affiliated network dentist.
- The QCD affiliated general dentist will charge for all services by code according to your QCD Schedule of Program Fees. Any procedure not listed will be charged at a 20% discount from the dentist�s usual and customary charge.
- The QCD affiliated specialty care dentist will provide a 20% discount for all services.
- Please pay for all services at time of service and your QCD visit is complete!
How do I select a dentist for services under the QCD program?
A list of the dentists for services under the QCD program is available for you to obtain in three different formats:
- See our website at www.qcdofamerica.com and go to the Affiliated Dentist section. There is an area for you to enter your zip code and the desired mileage radius that fits your specific needs. A list of the twenty (20) closest locations according to your parameters will be displayed.
- Contact your Human Resources Department for a copy of the QCD Affiliated Dentist Team for your area. This will feature all QCD locations in the area.
- Contact the QCD Corporate office at 972.726.0444 or 1.800.229.0304 and we will send you a copy of the Affiliated Dentist Team in your area.
Must I select only one dentist for service under the QCD Program?
The QCD Program allows you to select any dentist from our large network in your area. A different dentist may be selected for any member of your family and you may change dentists at any time without notification to QCD.
If you require services from a QCD affiliated specialty care dentist, simply select the specialty care dentist of choice from the QCD affiliated dentist team and make your appointment. There are no referrals necessary from your general dentist!
What if I need assistance in selecting a QCD dentist and making my appointment?

What if my QCD affiliated dentist�s bill does not seem to match my schedule of program fees?
Please contact our Membership Services Department immediately if you have a concern regarding estimated costs or your actual bill. The QCD representative will perform an evaluation to make sure that all charges are correct. In the event of an error, QCD will contact the dentist on your behalf to make sure that any necessary adjustments are made.
Can I refer my dentist to the QCD Affiliated Dentist Team?
Yes! Contact the QCD Membership Services Department and put in your request. We will contact the dentist and if contracted, the name will appear on the Newly Affiliated Dentist area of the web site. Our average time to contract a new dentist is 60 days.
How can I get assistance with my claims?
Please contact our claims administrator - IHC Health Solutions at 800.227.7620. If you need further assistance, please call our Membership Services Department at 972.726.0444 or 800.229.0304. If you have submitted a complete claim with all required information, you can expect your payment within no later than 10 business days. Our representatives will analyze your situation and make sure that all charges are correct.
How can I get assistance in using my vision benefit?
Your QCD Advantage Simple Savings Vision Care is provided at no additional cost by Davis Vision. The QCD Enhanced Vision Care (if purchased separately) is provided by Davis Vision and serviced by IHC Health Solutions � 800.227.7620. Please see the Davis Vision web site for a listing of vision providers in your area www.davisvision.com.
