Larry N. Sims

President and CEO

Mr. Sims has served as the President and CEO of QCD of America, Inc.® since the company began operations in 1992. Mr. Sims is a graduate of Baylor University, 1980.

Under Mr. Sims, QCD of America has been a pioneer in the dental and vision benefit market with unprecedented product innovation. QCD has set many new standards in the industry, including:

• The original No Charge for Employee Only coverage discount program

  This allows employer groups to provide a discount dental and vision program to all employees for NO COST.

  Employees pay monthly membership costs to cover dependents only.

• The All - American Dental and Vision Benefit Program for Employer Groups

  This allows the employer to offer up to four choices for employees at time of enrollment.

  This allows each employee to select the dental benefit program that satisfies their individual needs and the unique needs of their families.

  This allows all employees to afford to participate in dental and vision benefits in today's rising cost environment.

• The original and only dental employee benefit program that provides 100% reimbursement for almost all services - including major care

  In old style dental benefit plans, major services are covered at only 50% of the insurance company's usual and customary cost. This causes the employee to incur a large out-of-pocket cost that is not reimbursed - Not a good value for the premium paid.

  QCD members benefit from affordable premiums and reimbursement for out-of-pocket cost - this gives the member a much higher value for the premium paid.

MICHAEL D. HORDIES

VICE PRESIDENT OF OPERATIONS

Mr. Hordies has served as the Vice President of Operations for QCD of America, Inc. since 2001. Mr. Hordies is a native of Pittsburgh, Pennsylvania and has resided in the Dallas area since 1980.

Mr. Hordies is responsible for all Membership Services functions, Client Services functions and Accounting Functions.

Morris D. Hopkins

(1936 - 2007)

In memory of our beloved friend and Vice President of Administration.

Mr. Hopkins was a major part of the QCD organization from inception and will always be remembered by those that he touched in life.
